Rule 2.6. Form of Pleadings
Every pleading must have a caption with the court's name, a title, and a file number. The title to the petition must name all the parties; the title of other pleadings, after naming the first party on each side, may refer generally to the other parties.
Every pleading, motion and other document must be signed by at least one attorney of record or by the party personally if the party is unrepresented and contain the signer's address, e-mail address (if signer has e-mail) and telephone number. The court may strike an unsigned filing unless the omission is promptly corrected.
