Rule 4. Cover Sheets and Pleadings
A completed Cover Sheet shall be included with the initial pleading filed in all cases, except traffic cases.
If a case is refiled after previous dismissal this information shall be disclosed on the Cover Sheet and to the Court Clerk at the time of refiling.
An attorney shall include his or her bar association number, name, address and phone number on all pleadings and on decrees, orders and journal entries approved by counsel. The name of the attorney actually signing any pleading or dismissal must be legibly printed thereon. Copies of all pleadings filed subsequent to the petition shall be mailed or hand delivered to the assigned judge.
