Twenty-Fifth Judicial District Court Rule 18
Rule 18. Default Judgment
A party seeking a default judgment shall present the following matters to the Judge:
(a) Compliance with District Courts of Oklahoma Rule 10, Title 12, Chapter 2.
(b) The court file of the case.
(c) Evidence to show service on the defaulting party.
(d) Evidence to support the dollar amount of unliquidated damages.
(e) The journal entry of default judgment. Such journal entry shall reflect the actual default proceedings, i.e., it should not state witnesses testified, if they did not.
