Field-Initiated Ideas for Individuals who are Medically Fragile
Four Field-Initiated ideas (FII) grants of up to $40,000 per grant for 12 months will be funded by the DDPC to enhance care coordination, decrease barriers and promote inclusion into the community for individuals who are medically fragile. A “child or Adult who is Medically Fragile” means any individual who, because of an illness, congenital disorder, accident abuse, or neglect, has been left in a medically stable condition, but dependent on life sustaining medications, treatment, equipment and total assistance with activities of daily living. Individuals must also meet the criteria for the Federal Definition of Developmental Disabilities. The applicant’s program and budget narratives must provide the justification for the grant amount requested.
DDPC had identified the following areas of need for caregivers and individuals who are medically fragile. Proposals submitted for this RFP should focus on at least one of the identified areas of need below:
AREAS OF NEED
1. Systems Entry
• Reaching new caregivers and families before hospital discharge;
• Linking caregivers with other experienced caregivers;
• Care coordination strategies leading to reduced hospitalizations; and
• Ensuring quality care including recruitment and retention of in-home medical personnel (e.g. nurses).
2. Systems Navigation and Transition
• Navigation into and through health, education and disability service systems;
• Linkages between health and service providers including neonatology and intensive care units, traumatic brain injury treatment centers, and/or rehabilitative units;
• Family-centered care models;
• Health care planning; and
• Improvement of transition points across the age span and systems:
a. early childhood programs to school-aged programs,
b. inpatient to outpatient programs,
c. secondary to post-secondary school programs,
d. school-based transition to employment,
e. movement to a lesser restrictive residential setting.
3. Community Integration
• Sharing strategies and information about accessing natural and community-based supports;
• Developing, enhancing, and expanding community-based family and professional partnerships;
• Demonstrating innovative, cost-effective community-based supports (e.g. respite and other appropriate options) for families and caregivers;
• Developing innovative community-based residential options beyond nursing homes, and other long-term care options; and
• Creating models to enhance individual participation in community life including school religious, civic, and other activities.
4. Other
• Education of primary and secondary medical care providers and/or school personnel;
• Self-Advocacy skill development and linkages to other self-advocates;
• Family Supports including training on accessing community resources, bereavement and recovery, end of life care, palliative care, or siblings supports; and
• Professional development leading to enhanced supports for individuals who are medically fragile.
The full RFP can be downloaded from www.ddpc.ny.gov. All interested applicants are required to submit a one page letter of intent by October 26, 2012 to [email protected]. Letters of intent are required for applications to be accepted for this RFP.
It is the applicant’s responsibility to check the DDPC website periodically for any amendments to the RFP. All changes will be posted by November 13, 2012 at http://ddpc.ny.gov/Pages/requests_for_proposal.html. No other notifications will be given.
All final grant applications should be sent to the address listed above and to the attention of Sheila Carey, Executive Director, and must be delivered to the DDPC office by close of business (5:00 pm) on December 4, 2012. No applications will be accepted after this deadline.
