Quality Youth Development Programs After School
Objective: The New York State Office of Children and Family Services (OCFS) has revised the key dates identified below in the Advantage After School Program Request for Proposal (RFP) and the Bid Solicitation Announcement. NYS OCFS invites incorporated non profit organizations in partnership with a school (s) to submit proposals for the creation of quality youth development programs directly after school.
Revised Notes: The following revised deadlines and dates have been established: Deadline for submitting questions 08/10/07; date Frequently Asked Questions will be posted to the Online Bidder's List 08/15/07; date applications due 8/31/07 — note that applications must be postmarked no later than 8/31/07 and faxed or e-mailed versions will not be accepted, anticipated award announcement date 9/28/2007; and projected contract start date 3/1/2008.

The New York State Office of Children and Family Services (OCFS) invites not-for-profit community-based organizations and faith-based organizations to submit proposals for the development of quality youth development programs after school based on the Advantage After School Program model. Advantage After School Programs, in partnership with local schools, offer a broad range of educational, recreational, cultural age-appropriate activities in a safe environment. Advantage After School Programs integrate what happens in the school day with less formal learning experiences and encourages the active participation of children, youth and families in the design and delivery of program activities. The program must be available at a minimum three hours after school each day. The expectation is that these programs will enroll a group of children and youth who will attend daily. Programs must meet all applicable State Laws and Regulations including the registration process for School Age Child Care (SACC) as outlined in OCFS Regulations, part 414.
The award range is $50,000 to $300,000 with a maximum cost per child of $1,250 per year. The projected contract start date is March 1, 2008. Awards will be made for twelve months with one-year renewals for an additional four years, subject to availability of funding and satisfactory contractor performance. Proposals must be postmarked by August 31, 2007. Faxes or e-mail submission will not be accepted. Awards provided by the Advantage After School Program RFP are funded under Federal Temporary Assistance to Needy Families (TANF). TANF funds may be used for the direct delivery of services and activities to elementary, middle, and high school-aged youth to improve their life skills and assets so as to reduce the incidence of teen pregnancy, and other health compromising behaviors. Funds for training staff, parent participation, recruiting and retaining youth, program scheduling, daily program coordination, staff supervision and youth involvement in the design and delivery of the program are considered direct service activities for this program. TANF funds may not be used for major renovations, capital construction or transportation. Motor Vehicle Theft and Insurance Fraud Prevention Demonstration Program
Subject to availability of funding, the NYS Division of Criminal Justice Services (DCJS) is issuing a Request for Proposals (RFP) to solicit applications from local government and non-for-profit agencies for the Motor Vehicle Theft and Insurance Fraud Prevention Demonstration Program. Approximately $5,300,000 will be available. The RFP is available on the DCJS website at http://www.criminaljustice.state.ny.us/ofpa/newrfp.htm. Proposals will be accepted through September 13, 2007.

NURSING HOMES LICENSED UNDER ARTICLE 28 OF THE PUBLIC HEALTH LAW
On-Time Quality Improvement for Long Term Care
Eligibility is limited to nursing homes licensed under Article 28 of New York's Public Health Law with pressure ulcer rates for high risk residents that average 10% or more the first two quarters of 2007, as measured by NYS Department of Health and posted on its website on the Nursing Home Profile page (http://nursing homes.nyhealth.gov/). Nursing homes are eligible if they have current health information system (HIT) for certified nurse aid (CNA) documentation or if they do not.
Nursing homes awarded funds under this RFA may also apply for and receive funds under any other pressure ulcer reduction RFA issued by the Department of Health. However, a nursing home that receives an award under this RFA to implement the On-Time model must propose a different project in its application for funds under any other pressure ulcer reduction RFA issued by the Department of Health.
On-Time Quality Improvement for Long Term Care
The Division of Quality and Surveillance (DQS) announced the “War on the Sore” in a Dear Administrator Letter (DAL) #06-22, November 30, 2006. Since the release of this DAL, DQS has worked closely with Nursing Homes, provider associates, researchers and the federal government to identify evidence- based quality improvement models that produce sustainable decreases in prevalence and incidence of pressure ulcers among nursing home residents. One such evidence-based model is the “On-Time Quality Improvement for Long Term Care”, developed by Dr. Susan Horn and colleagues at the Institute of Clinical Outcomes Research and additional colleagues at Health Management Strategies, with financial support from the federal Agency for Healthcare Research and Quality (HARQ) and the California Health Care Foundation. It has been implemented, tested and refined in 30+ nursing homes across the nation. This approach to nursing home acquired pressure ulcer prevention has produced an average decrease of 33% in prevalence rates in the nursing homes that implemented the model.
The On-Time model integrates clinical guidelines and clinical information into each nursing home's daily routines and processes for assessment, care planning, care delivery, communication and reassessment using health information technology (HIT). The model facilitates timely information flow that informs weekly monitoring of resident status and on-going care planning. The communication mechanisms used in the On-Time model are effective and efficient and provide staff with current and accurate information on the resident on a weekly or more frequent basis.
Up to 12 nursing homes that commit to implementing the model for all of their chronic or long stay beds will be selected to participate. Six hundred thousand dollars ($600,000) has been allocated to this initiative. Awards from the Department of Health are capped at $49,999 and are intended primarily to subsidize the costs of the required health information technology that must be purchased to implement the On-Time model. The Department will execute a two-year contract with each nursing home awarded funds under this initiative.
The anticipated RFA release date for On-Time Quality Improvement is: August 1, 2007.
In lieu of an applicant conference, the Department is offering interested nursing homes an interactive live webcast on August 8, 2007, 1:30- 3:30 pm ET. Those who wish to participate in the webcast must register at http://www.delmarvafoundation.org/events/AHRQ/web Cast 2/. There is no cost and no limit on the number of staff that a nursing home can register for the webcast. The developers of the On-Time Quality Improvement model and staff from nursing homes that have implemented it will present additional information on the model during the webcast. There will also be multiple opportunities for nursing home staff to ask questions about the On-Time model and the RFA.

Shared Municipal Services Incentive (SMSI) Awards
The NYS Department of State is announcing a competitive awards program to provide grants for cooperative efforts by two or more entities that achieve savings through the sharing of services for the following:
Local Health Insurance Incentive Awards:
A total of $1.325 million is available to support costs associated with the creation of municipal health insurance consortiums and to match savings achieved by joining the State's health insurance program, or to provide collective bargaining incentives that promote employee cost sharing of health insurance premiums. Municipalities may apply for awards of up to $500,000. Matching funds are not required for these projects.
