Use this verdict form for each defendant, whether there are one or more counts. For more than one count, repeat the paragraph using the bracketed phrase each time, filling in the appropriate count number. For more than one defendant, use one verdict form for each defendant. Fill in all blanks except for “guilty” or “not guilty.”

In the caption, use the title “Verdict Form” rather than “Verdict Form A.”

Use WPIC 151.00 (Basic Concluding Instruction), with this verdict form.
