§ 1005. Application for approval by department
The incorporators shall make an application to the department for approval of the proposed institution in a manner prescribed by the department and shall deliver to the department when available:
(a) the articles of incorporation;
(b) the affidavit required by section 1003;1
(c) evidence of reservation in the Department of State of the name of the proposed institution;
(d) applicable fees payable to the department in connection with the articles and with the conduct of the investigation required by section 1007;2 and
(e) as soon as available, proof of publication of the advertisement required by section 1006.3
