§ 240.12A. Insurance Department (Adm. Code § 612-A)
The Insurance Department is authorized to charge fees for the following purposes and in the following amounts:
(1)
Insurance companies, associations or exchanges
(i)
Valuation of life insurance policies based on a per thousand dollar value of such insurance

Filing copy of charter or amendment of a domestic, foreign or alien company, association or exchange
