Any person desiring to take the examination for a license to practice veterinary medicine shall make application at least thirty days before the examination on a form provided by the board. The application shall be accompanied by such evidence as is required to show the eligibility of the applicant. All applications shall be in accordance with the rules of the board and shall be signed and sworn to by the applicant.
