Family Support Services Funds Program
The Finger Lakes DDSO is pleased to advise Not-for-Profit agencies of the availability of Family Support Services funds effective April 1, 2008. The Family Support Services program helps families caring for a child or relative with a developmental disability living at home. These services are able to assist families, caregivers and the consumer to enhance family stability and preserve family unity. The driving philosophy of family support is to take a responsive, equitable and accessible approach to providing the services needed to support individuals with a developmental disability living with their families in their own homes, ensuring consumer choice, individual involvement and in an inclusive setting.
The funds available are to help address the needs of individuals with developmental disabilities especially those with challenging behaviors. The total amount of available funding for the Request for Application (RFA) is $700,000, including a recommendation that 25 % of this allocation ($175,000) be used to provide funding to enhance existing programs in the area of crisis intervention and for individuals with challenging behaviors. Providers applying for funds will need to clearly describe in measurable criteria the individuals they will serve by describing specific indicators of their challenging behaviors. Indicators of challenging behaviors may include self-injurious or assaultive behaviors, involvement in the criminal justice system, have a DDP-2 profile with a high-end behavior score, require a 1:1 aide in the classroom due to behavior issues, or whether the child/family has had crisis intervention services within the past two months.
Based on the recommendations from the Family Support Services Consumer Council the following areas have been identified as funding priorities taking into consideration that programs are inclusive in nature including program enhancements to existing Family Support Services programs. It is expected that these programs will meet the behavioral challenges of the individuals to be served. The funding priorities include: in-home respite programs with adequate and trained staffing to meet the behavioral needs of the child served; wrap around services (before and after school care including school vacations, weekends and holidays); recreation programs with staff trained to work with individuals with challenging behaviors and at a staffing level to meet the needs of the program participants; parent support groups; enhanced transportation opportunities to FSS programs; and any other innovative programs.
The funding will be available to serve the following counties: Monroe, Chemung, Livingston, Schuyler, Steuben, Wyoming, Ontario, Seneca, Wayne and Yates.

FERTILITY CLINICS
Infertility Demonstration Program
Description: It is anticipated that approximately $10 million will be made available via contracts to fund in vitro fertilization (IVF) and gamete intrafallopian transfer (GIFT) procedures for infertile women who meet certain age and other eligibility requirements. Funds will be made available to fertility clinics meeting the following eligibility criteria, as published in Centers for Disease Control and Prevention (CDC) 2004 Assisted Reproductive Technology Success Rates data. To be eligible, clinics must be NYS licensed tissue banks, and, either be current grant program participants or have reported to CDC in 2004 that they performed over 100 IVF cycles, with a success rate of greater than 30 percent live births per fresh cycle for women less than 35 years of age. All eligible providers received a direct letter of invitation to participate, and all interested and eligible providers will be awarded funding upon successful completion of the contract process. The letter of interest sent to eligible providers also requested their interest in participating in an additional male subsidy, using a set aside of $500,000 for TESE services.
If additional funds are made available during the contract period, they will be distributed in proportion to their initial awards, which are primarily based on cycle volume, with an upstate adjustment factor to balance the paucity of eligible upstate providers.

Contract Term: Funding will be awarded for a period of one year, with a one year extension possible depending on performance and availability of unexpected funds.

GRASS ROOTS, FREESTANDING, NOT-FOR-PROFIT ORGANIZATIONS UTILIZING A LARGE NUMBER OF VOLUNTEERS, THAT OFFER A BROAD RANGE OF EDUCATION AND SUPPORT SERVICES FREE OF CHARGE, FOR INDIVIDUALS DIAGNOSED WITH BREAST CANCER*
Community-Based Breast Cancer Education, Counseling, Outreach and Support Services
*For purposes of this RFA, a grass roots, freestanding organization is defined as a community-based organization which is not part of or affiliated with a statewide, national or international organization or a major medical or academic institution.
Applications will be accepted through February 2, 2008 for grants to support grass roots, freestanding community-based programs that provide a broad range of services such as counseling, education, outreach and support for individuals diagnosed with breast cancer in New York State (NYS)with the overall goal of helping to the reduce stress experienced by individuals diagnosed with breast cancer. The provision of such services can improve their ability to cope and deal with the uncertainties, challenges and life complications that accompany the disease.
The Department of Health currently expects that approximately 10 awards will be made as a result of the application process. It is expected that awards will be made to support both the infrastructure and reimbursement for counseling, education, outreach and support services provided. Current plans are to issue approximately 10 awards. Each of these contracts will fund up to $40,000 for the first year of the contract and $20,000 per year for each of the subsequent four years of the contract.

Occupational Safety & Health Training and Education Program (OSH T&E)
The Hazard Abatement Board announces the availability of funds for the development and implementation of programs promoting occupational safety and health in the workplace through training, education, and other proven preventative programs. Total available funds anticipated for programs under Article 29 of the NYS Labor Law will be approximately $6.5 million. Applicants may apply for any portion thereof. Grants awarded will be based on the number of quality programs approved and the availability of funds. Request for Proposals are available as of December 5, 2007. OMRDD “Quality Improvement Demonstration Program”
Applications will be accepted through February 1, 2008 for projects from non-profit or public agencies that serve the MRDD population to develop quality improvement demonstration projects that benefit direct support workers. Proposed projects should pertain to one or more of the following activities:
Implementing commercially developed or agency-developed training for direct support workers and/or frontline supervisors,
developing and implementing mentoring programs for direct support staff and/or their supervisors,
enhancing direct support staff recruitment practices using Realistic Job Previews,
developing and implementing programs to train or otherwise support self-directing individuals in the recruitment, training or supervision of their direct support staff,
undertaking initiatives to improve agency culture and workforce management practices in relation to direct support staff.

2007–2008 Legislative Initiatives — Homeless Advocacy Program
Type of Recipients: Not-for-profit entities: Barrier Free Living, Inc., Coalition for the Homeless, Bushwick Economic Development Corporation, Citizens Advice Bureau, Henry Street Settlement, Urban Pathways, Inc., Weston United Community Renewal and West Side Federation for Senior and Supportive Housing.
Description of Programs: 2007–2008 Legislative Initiatives -Homeless Advocacy Program assists disabled homeless persons to obtain supplemental SSI or Social Security Disability benefits to which they are entitled, also assists persons seeking permanent housing; Emergency Homeless Needs meets the emergency needs of those homeless and those at risk of becoming homeless by providing services such as emergency grants for rent arrears, crisis intervention, emergency feeding, or summer youth services for homeless services; Homeless Shelter Health and Safety Conditions includes emergency capital improvements, case management enhancement and operating supports to improve safety, health and sanitary conditions in adult and family homeless shelters; and Supportive Homeless Outreach supports homeless outreach services in NYC including expanded services in the Bronx, Brooklyn and Queens, also supports mobile outreach teams to provide advocacy and support services for street-bound homeless.
Reason for Exemption: Consistent with Chapter 862 of the Laws of 1990, OTDA requested an exemption from initially publishing this contract opportunity in the procurement opportunities newsletter, i.e., the NYS State Register. In accordance with Section 144(2)(3) of the Economic Development Law, the Office of the State Comptroller has granted the exemption. OTDA must contract with the named not-for-profits in order to continue to fund these valuable programs. A total of $1,405,000 has been allocated in the budget to fund these contracts beginning April 1, 2007. No applications will be accepted.
