Hazard Mitigation Grant Program
The State Emergency Management Office (SEMO) is requesting Letters of Intent (LOI) from the eligible applicants, for the development of mitigation plans. Funding will be available under the Hazard Mitigation Grant Program (HMGP) from three major disaster declarations—FEMA-1665, FEMA-1670 and FEMA 1692-DR-NY. These declarations were made subsequent to the October 2006 snowstorm that impacted Erie and several other western New York counties, the November 2006 flooding that impacted several counties in the Southern Tier-east and central parts of the State and the “Nor'easter” that affected the New York Metropolitan region of the state.
Via the regulations governing the use of HMGP funds, Planning grant funds will be used to support the development of multi-hazard single or multi-jurisdiction hazard mitigation plans. It may also be used to support the required update needed to maintain currency. FEMA-approved mitigation plans are required to be in place before eligible applicants can apply for project funding. Only those Counties and other local governments that do not currently have FEMA-approved hazard mitigation plans or are currently engaged in such planning can apply for these funds.
The LOIs will be accepted through January 2, 2008 and must be submitted by eligible applicants who include state and local governments; private non-profit organizations or institutions that own or operate a private non-profit facility; or Indian tribes. It is imperative that LOIs be submitted as soon as possible because FEMA requires the funds be committed within one year of the disaster declaration. The first anniversary of 1665 is 10/24/06; 1670 is 12/12/06 and 1692 is 4/24/07. Integrated Cancer Services Program, Breast, Cervical and Colorectal Cancer Screening Partnerships, (CSP Partnerships)
Applications will be accepted through December 7, 2007 for grants to support community-based programs that provide uninsured and underinsured men and women in New York State (NYS) with routine screening for breast, cervical and colorectal cancer, with the overall goal of reducing the mortality rates for these diseases in NYS. These community-based programs (known as Cancer Services Program partnerships, or CSP partnerships) will coordinate recruitment for and provision of comprehensive, age-appropriate breast, cervical and colorectal cancer screening services, follow-up care and other related services to eligible men and women.
The Department of Health currently expects that approximately 44 to 48 awards will be made as a result of the application process. It is expected that awards will be made to support both infrastructure and reimbursement for clinical services provided to eligible men and women through three separate contracts, as follows: one New York State (NYS) contract to support infrastructure for a one-year period, renewed annually for up to five years, a second, five-year NYS contract to support reimbursement for clinical services, and a separate, third contract with Health Research, Inc. (HRI) for reimbursement of clinical services, renewable annually over a five-year period. It is expected that the maximum infrastructure award to any applicant for the first year will be $345,000.

APPROVED ASSISTED LIVING PROGRAM; HOME CARE SERVICES AGENCY
1584 Assisted Living Program (ALP) Beds
As a result of an amendment to the Social Services Law adopted as part of the state budget process for SFY 2007-08, the New York State Department of Health is soliciting applications for 1584 Assisted Living Program (ALP) beds. In order to be included in the selection process, completed applications must be received by the Department or be postmarked on or before December 14, 2007.
The 1584 ALP bed distribution will be statewide. The statewide ALP bed allocation will be determined based on the US Census projections for the year 2015, taking into consideration the number of currently licensed ALP beds and counties without any licensed ALP beds.
An application package and explanatory materials will be made available at the Department of Health website at www.health.state.ny.us on September 27, 2007.
Eligible Applicants
An approved Assisted Living Program is an entity comprised of:
1. A certified Adult Care Facility
(a) Adult Home (AH) OR
(b) Enriched Housing Program (EHP) AND
2. A Home Care Services Agency
(a) Licensed Home Care Services Agency (LHCSA), OR
(b) Certified Home Health Agency (CHHA), OR
(c) Long Term Home Health Care Program (LTHHCP).
Applicants who operate an Assisted Living Program must be either a not-for-profit corporation, a non-publicly traded business corporation or limited liability company, a public agency, or an individual or group of individuals acting as partners.
These entities must either already hold the required certificates, or have an appropriate application in process, or request such certification as part of the application for approval as an Assisted Living Program.
The ALP application allows an entity to apply simultaneously for certification as an adult home or enriched housing program and a licensed home care services agency.
The entity authorized to operate an Assisted Living Program must be certifiable as an adult care facility and as a home care provider. Applicants to operate an Assisted Living Program must be one of the following: a not-for-profit corporation, a business corporation (that is not publicly traded, and whose shares are owned by natural persons), a limited liability company (if members are corporations or limited liability companies, the shareholders or members of same must be natural persons), a public agency, or an individual or general partnership comprised solely of natural persons.
These entities must either already hold the required certificates, or have an appropriate application in process, or request such certification as part of the application for approval as an Assisted Living Program.
The requirement for an “eligible applicant” can be met in one of two ways. Either by (1) a single legal entity which possesses or is eligible to apply for an adult home or enriched housing program operating certificate and for approval of home care services; or by (2) one or more legal entities with identical ownership that in combination, possess or are eligible to apply for, an adult home or enriched housing program operating certificate and for approval of home care services.
This is a competitive application review process. Applications will be scored on completeness and other factors with the highest consideration given to:
Projects targeted toward services to people who otherwise may be inappropriately placed in a nursing home and whose residential and healthcare needs can be met by the ALP, such as the frail elderly or physically disabled;
Applicants who demonstrate commitment to admit and retain individuals in receipt of Supplemental Security Income (SSI), Safety Net benefits or Medical Assistance (MA);
Proposals that increase the supply of new ALP beds rather than convert existing adult home and/or enriched housing program beds to ALP beds; and
Applicants who are able to commence services most quickly by demonstrating completion or near completion of financial arrangements, site control and those who meet architectural compliance without any new construction or renovations, ideally operational within 22 months of contingent approval.
The order in which the above bulleted review items are listed is NOT an indicator of the level of scoring weight applied to these items.
