(1) Health care scholarship shall mean Regents health care professional opportunity scholarships awarded pursuant to Education Law, section 605(10).

(2) Professional scholarship shall mean a Regents professional opportunity scholarship awarded pursuant to Education Law, section 605(10).

(1) Recipients of such awards shall annually file with the corporation an application for payment of an award in accordance with section 2201.1(a) of this Title and further, shall report to the corporation on such forms and at such times as the president may prescribe, all information required by the president, including but not limited to anticipated date of graduation. In the event a recipient fails to file such annual application in any academic year in which he or she may be eligible, the recipient shall file a report, on such forms and at such times as prescribed by the president, regarding his or her status during the academic year in which an application for payment is not made.

(2) Recipients of health care scholarships and professional scholarships shall be enrolled in an approved program, as defined by the commissioner pursuant to section 145-6.1(b)(1) of this Title.

(1) Annual health care scholarship payments shall be made in an amount of up to $10,000; provided, however, that the award shall be reduced by $1 for every $4 by which the applicant's income shall exceed the amount set forth for an economically disadvantaged student pursuant to section 27-1.1 of this Title; provided, further, that such reductions shall not reduce an annual award to less than $1,000. Anything to the contrary herein notwithstanding, however, no award shall exceed the costs of attendance.

(2) Annual professional scholarship payments shall be made in an amount of up to $5,000; provided, however, that the award shall be reduced by $1 for every $4 by which the applicant's income shall exceed the amount set forth for an economically disadvantaged student pursuant to section 27-1.1 of this Title; provided, further, that such reduction shall not reduce an annual award to less than $1,000. Anything to the contrary herein notwithstanding, however, no award shall exceed the costs of attendance.

(1) Health care scholarship recipients shall be entitled to payments of awards for up to but not more than four academic years.

(2) Professional scholarship recipients shall be entitled to payments of awards for up to but not more than four academic years, unless enrolled in a program of study recognized by the Regents as normally requiring more than four years to complete, in which case the recipient shall be entitled to payment of awards for that number of academic years as recognized by the Regents as required.

(1) A recipient entitled to file a further application for an award under this section who fails to file such application in any academic year in which he or she may be eligible, shall file a report, on such forms and at such times as prescribed by the president, regarding his or her status during the academic year in which an application for payment is not made.

(2) All other recipients of awards under this section shall report to the corporation, on such forms and at such times as the president may prescribe, such information as to his or her graduation, licensure or certification, and employment status, as the president may require.

(1) Failure of a recipient to comply with the service condition set forth in Education Law, section 605, shall entitle the president to recover from such recipient an amount determined by the provisions set forth in that statute.

(2) Repayment of the amount payable shall be made in equal monthly installments of principal and interest so as to complete repayment within 10 years, or such lesser period as determined by the president taking into consideration the amount owed and the circumstances of the recipient; provided, however, that in no event shall the monthly payments be less than $50 plus interest. A written agreement of repayment shall be obtained for each recipient, which such agreement shall include such terms and conditions as shall be deemed appropriate by the president.
