(a) A request for review of a claim or prior approval request denial will be considered to have been withdrawn under the following circumstances:

(b) A request for review will be considered to have been abandoned if neither the enrollee nor the enrollee's authorized representative appears at the hearing unless the enrollee or the enrollee's authorized representative provides the fund administrator, within 30 days of the scheduled date of the hearing, a good cause explanation for such failure to appear.
