All applicants for positions must complete an application for employment and a statement of qualifications on form DSS-800 or MSD-330 to be kept on file in the local social services department for a period of not less than one year, except that the application for employment of any person actually employed shall be retained in the permanent personnel record of such employee in the form and manner prescribed by the department.
