459.4 Program requirements.
(a) A senior citizen center shall be eligible for reimbursement for the following services:
(1) information and referral services, including:
(i) assessment of the individual situation and problems of the senior citizen who requires or seeks information or advice with respect to his needs, especially health, consumer information, employment, housing and protective services;
(ii) liaison with agencies providing services in order to make them available to individuals using the centers; and
(iii) effective coordinative relationship by voluntary senior citizen centers with local social services districts;
(2) nutrition (congregate meals): at least one hot meal per day provided during five days per week with consideration being given to the nutritional and health requirements of individuals;
(3) counseling services, including short-term counseling by qualified staff to improve social functioning;
(4) employment counseling: matching of individuals looking for work to specific employment opportunities;
(5) group activities, including recreational programs, educational programs, opportunities for self-improvement through group activities and community-oriented projects;
(6) transportation to and from user's home and facilities, as well as to other services provided in the community;
(7) health maintenance services, including blood pressure checks, annual physical examinations, referrals for visual or hearing impairment, and interpretation of effects of drugs;
(8) community service volunteer opportunities: provision of opportunities to participate in volunteer and paid service roles with various community agencies and organizations;
(9) leadership development: developing an individual's group leadership skills and personal effectiveness in dealing with others;
(10) facilitation of other agencies' services by maintaining effective liaison with other public and private community agencies in order to make their services more accessible to the individuals at the senior centers;
(11) advocacy: insuring receipt of entitlements by those eligible for them; and
(12) outreach: public information through direct contact with potential users to advise them of available services; and dissemination of information to promote individual and community awareness, and usage of center services by eligible nonparticipants.
(b) A senior citizen center shall be required initially to provide as a minimum:
(1) one meal per day; and
(2) the following services as defined in subdivision (a) of this section:
(i) information and referral;
(ii) group activities;
(iii) counseling; and
(iv) outreach.
(c) Each annual contract entered into with a senior citizen center shall have a plan which will delineate such center's efforts toward progressive implementation of the additional program requirements specified in subdivision (a) of this section. Such implementation may be contingent on the availability of additional public or private funds to the center.
