(a) The plan becomes effective on the date so designated by the commissioner once the plan is approved by the commissioner, and remains in effect until such time as the commissioner approves a plan for the next three-year period.

(b) Any plan submitted to the department must be modified by the approved annual implementation report. Amendments to the plan may be made in accordance with section 407.10 of this Part.
