(a) Requests for payroll information (name, business address, title and salary of officers and employees) about officers and employees at the central office of OPWDD shall be made in writing, on the form prescribed by the State Comptroller, to the business officer at 44 Holland Avenue, Albany, NY 12229.

(b) Requests for payroll information (name, business address, title and salary of officers and employees) about officers and employees employed at OPWDD facilities shall be made in writing, on the form prescribed by the State Comptroller, to the business officer of the facility at which such person is employed. The addresses of such business officers are set forth in section 603.4(a)(2) of this Part.
