Pursuant to the authority conferred by the Public Health Law (cf. § 4173 above), the Department of Mental Hygiene and its institutions are entitled to a fee for such service and will, accordingly, charge those entitled to such records, on request, a fee of two dollars for each verified transcript of a death record. One dollar shall be charged for each certified copy of a certification (commitment) or other admission paper. An additional fee of two dollars shall be charged for a search of the file for each patient. For the preparation of proofs of death by physicians for the use of insurance companies a charge of one dollar shall be made. Similarly, a charge of one dollar shall be made for the service of legal papers upon patients. Payments in excess of two dollars may be accepted when tendered. All such fees must be paid into the State treasury.
