12 CRR-NY 81.1
81.1 Application requirements.
(a) A labor organization desiring to register and file with the Department of Labor any device adopted for the purpose of designating the product of the labor of its members shall make application on a form supplied by the Industrial Commissioner. Together with the application, the labor organization shall submit three copies of the device of the same size as is used on or in connection with the goods, wares or merchandise worked on or produced by the members of the labor organization and three photostatic enlargements thereof five inches by seven inches. All six copies, both enlarged and unenlarged, wherever possible, should be pasted on or attached to separate sheets of white paper 8½ inches by 11 inches.
(b) The president or chief executive officer or the secretary of the labor organization shall sign the application.
(c) The application must be accompanied by a non-refundable filing fee of $10.
