(a) Conditions of participation shall be modified, as necessary, by the director.

(b) Any modification of the conditions of participation shall involve the youth and, to the extent feasible, the parent.

(c) All modifications shall be in writing and signed in the same manner as the original conditions of participation.

(d) A copy of the modification shall be given to the youth and to the parent.

(e) A refusal by a youth to consent to a modification shall result in termination of the youth's participation in a day placement program.
