(a) Requests for access to, correction or amendments of records shall be made in writing. All requests shall be filed with or referred to the appropriate personal privacy compliance officer.

(b) No special form is required for such requests. A request should reasonably describe the record sought. When possible, the requestor should supply identifying information to assist the department in locating the records sought.
