FORM 11-206. NOTICE INCLUDED IN CINA SUMMONS
TO THE PERSON SUMMONED: The Court, at this or any later hearing, may consider and enter orders concerning the shelter care, commitment, custody, treatment, and supervision of the respondent child __________ (full name of child); responsibility for the child's support; controlling the conduct of persons before the court; and assessing court costs.
You may hire a lawyer to represent you. If you do so, be sure to show this Summons to the lawyer. If you cannot afford a lawyer, promptly contact the Office of the Public Defender on any weekday between 8:30 a.m. and 4:30 p.m. at: __________. A postponement will not be granted because you fail to contact a lawyer.
If you do not want a lawyer but wish to subpoena witnesses on your behalf, you must request promptly issuance of the subpoenas. A postponement will not be granted if you fail to do so.
If you received a Request for Witness Subpoena Form with this Summons, you must list neatly the names and addresses of the witnesses you wish to call on the form and return promptly the form to the Clerk of the Juvenile Court at the address shown on the form.
If you did not receive a Request for Witness Form, you must contact promptly the Clerk of the Juvenile Court on any weekday between 8:30 a.m. and 4:30 p.m. at __________ (telephone number), who will provide you with the necessary subpoena forms.
Any reasonable accommodation for persons with a disability must be requested by contacting the court before the hearing.
