3. Child Support forwarded directly to the recipient and accepted is in direct violation of this Order.
4. The first of said child support payments shall be due and payable on __________.
5. This is not a wage assignment for payment of support. It is the responsibility of the __________ to make child support payments to the Friend of the Court.
