Rule 702. VS-300 Form
Currentness
The party filing the petition for dissolution of marriage shall tender to the clerk, at the time of filing, a completed VS-300 form typed and with signature in black or blue ink. Handwritten VS-300 forms will not be accepted. A blank VS-300 Form is available from the office of vital statistics website. https://chfs.ky.gov/agencies/dph/dehp/vsb/Pages/default.aspx.
