LRP 15. Information to be provided when child support may be ordered
(1) In any action in which a party may be ordered to pay child support, at least one party shall provide the Court with the information necessary to calculate child support pursuant to the child support guidelines and shall file a completed child support guidelines worksheet incorporating the information. The party providing the information to the Court shall verify, under oath, that the information is accurate and complete to the extent of the party's knowledge.
(2) If the parties agree upon a child support obligation that deviates from the child support guidelines worksheet, the parties shall verify to the Court, under oath, that they have each been advised of the amount indicated by the worksheet and whether any public assistance is being paid on behalf of the child(ren).
