This matter having come before the Court on the __________'s Motion for Rule of Contempt why the Petitioner/Respondent, __________, should not be held in contempt for __________, and the Court being sufficiently advised:
IT IS HEREBY ORDERED:
The Petitioner/Respondent, __________, shall appear before this Court on the ___ day of __________, 2012 at __________ at the Warren County Justice Center, 3rd Floor, Bowling Green, Kentucky; Judge __________'s courtroom; and show cause why he/she should not be held in contempt and sanctioned for failure to comply with the prior orders or judgment of this Court. Pursuant to Lewis v. Lewis, 875 S.W.2d 862 (Ky. 1993) the Petitioner/Respondent is hereby informed that the Court is considering incarceration as a sanction against him/her, and he/she is advised of his/her right to counsel. If the Petitioner/Respondent maintains that he/she is entitled to appointment of counsel as an indigent under KRS Chapter 31, then not later than __________, 20 ___, the Petitioner/Respondent shall file with the Clerk of this Court an Affidavit of Indigency and shall contact the Warren County Public Advocate, who shall appear with the Petitioner/Respondent at this show cause proceeding on __________, 20 ___, at __________ for possible appointment to represent the Petitioner/Respondent pursuant to Lewis v. Lewis, supra.
The Warren County Sheriff's office shall cause a copy of this Order to be served upon the Petitioner/Respondent by personally delivering same to him/her and shall make due return the Court evidencing such service.
In the event the Petitioner/Respondent, __________, fails to appear, a contempt finding will be entered and a warrant of arrest issues against the Petitioner/Respondent, __________.
