§ 595.4. Replacement Issuance.
A replacement Certificate of Registration card may be issued by the State Fire Marshal. A written statement describing the reasons for the replacement issuance shall be submitted by the registrant before replacement will be issued. The statement shall explain fully the reason for the destruction of the certificate of registration card. The fee as outlined in Section 560(d)(7) shall be submitted for the issuance of a replacement certificate of registration card.
