§ 30521. Application.
(a) An Application to participate in the Program shall be provided by the Commission and will be available through an online portal. Applications may be submitted via the online portal beginning September 1 for the following academic year. An Applicant shall submit the Application through the online portal to the Commission no later than April 1 of the Academic Year for which the Applicant is applying. (For instance, an Applicant applying for a grant for the 2023-24 Academic Year may submit an Application beginning September 1, 2022, but no later than April 1, 2024). The Applicant shall provide the following information to the Commission:
(1) Social Security Number,
(2) Last Name, First Name and Middle Initial;
(3) Address;
(4) Date of Birth;
(5) Phone Number;
(6) Email Address;
(7) Institution, enrollment status, and professional preparation program type;
(8) The applicant's signature under penalty of perjury, under the laws of the State of California, that all statements contained in the application and any accompanying documents are true and correct, with full knowledge that all statements made in the application are subject to investigation. Any false or dishonest answer to any question may be grounds for denial to participate in the Program and subject to prosecution under applicable state law. The applicant authorizes the exchange of any student records and any application information between the Commission and the program in which the applicant is enrolled. The applicant shall provide documentation to verify all information provided, if requested.
(b) An Applicant shall complete a Free Application for Federal Student Aid or California Dream Act Application for the Academic Year for which the Applicant is applying.
(c) An Applicant may receive a Pre-Enrollment Grant Eligibility Determination from Institutions listed on their FAFSA/CADAA. To receive a Pre-Enrollment Grant Eligibility Determination, the Application shall be submitted to the Commission beginning September 1 for the following Academic Year, but no later than 60 days prior to the program's enrollment deadline. The Pre-Enrollment Grant Eligibility Determination shall provide conditional information regarding the award amount that the student may be eligible to receive upon enrollment in a professional preparation program and verification of eligibility and award determination by the Institution.
(1) The Pre-Enrollment Grant Eligibility Determination is not a requirement to be considered for a Program award. Applicants who do not need such a determination may apply up until April 1 of the Academic Year for which they are requesting a Program award.
(2) An Applicant shall update their Application consistent with subdivision (a) (7) of Section 30521 if they choose to enroll at an Institution other than what is currently listed on their Application.

History
1. New section filed 2-10-2022 as an emergency; operative 2-10-2022 (Register 2022, No. 6). A Certificate of Compliance must be transmitted to OAL by 8-9-2022 or emergency language will be repealed by operation of law on the following day.
2. Assembly Bill 181 (Stats. 2022, Ch. 52, Sec. 97) amended Education Code section 69617 to change the effective period of emergency regulations adopted pursuant to Education Code section 69617(g)(1). Pursuant to Education Code section 69617(g)(2), a certificate of compliance must be transmitted to OAL by 6-30-2025 or the language adopted in the emergency order of 2-10-2022 will be repealed by operation of law on the following day (Register 2022, No. 29).
3. Amendment of section filed 1-23-2023; operative 1-23-2023. Exempt from the APA pursuant to Education Code section 69617, subdivision (b)(3), and submitted to OAL for filing and printing only pursuant to Gov. Code section 11343.8 (Register 2023, No. 4).
