§ 6501.5. Required Newborn Screening Forms.
Currentness
The following newborn screening forms from the Department shall be utilized in accordance with provisions in this Group:
(a) For all newborn screening specimen collection or religious refusal pursuant to § 6501.2, § 6505(a)(1), § 6505(a)(2), § 6505(b)(1), § 6505(b)(2), § 6505(c)(1), § 6506(a), § 6506.6(a), § 6506.6(b)(1) and § 6506.6(c)(1): CALIFORNIA NEWBORN SCREENING TEST REQUEST FORM (NBS-TRF) (CDPH-4409). This form is hereby incorporated by reference.
(b) For county registrars who register infants born outside of a perinatal licensed health facility or who are not attended by a birth attendant pursuant to § 6505(d): NOTIFICATION OF REGISTRATION OF BIRTH WHICH OCCURRED OUT OF A LICENSED HEALTH FACILITY (NBS-OH) CDPH 4460 (01/09). This form is hereby incorporated by reference.

History
1. New section filed 4-4-2016 as an emergency; operative 4-4-2016 pursuant to Health and Safety Code section 124977(d). Submitted to OAL for filing and printing only pursuant to Health and Safety Code section 124977 (Register 2016, No. 15). Pursuant to Health and Safety Code section 124977(d)(2), these regulations shall remain in effect until revised or repealed by the Department of Public Health.
2. Statement of Compliance pursuant to Health and Safety Code section 124977(d)(1) as to 4-4-2016 order with amendments filed 4-17-2017. Exempt from OAL review pursuant to Health and Safety Code section 124977(d)(2). Submitted to OAL for filing and printing only pursuant to Government Code section 11343.8 (Register 2017, No. 16). Pursuant to Health and Safety Code section 124977(d)(2), these regulations shall remain in effect until revised or repealed by the department.
3. Amendment of subsection (a), repealer of subsection (b) and subsection relettering filed 6-6-2019 as an emergency; operative 6-6-2019 pursuant to Health and Safety Code section 124977(d). Submitted to OAL for filing and printing only pursuant to Health and Safety Code section 124977 (Register 2019, No. 23). Pursuant to Health and Safety Code section 124977(d)(2), these regulations shall remain in effect until revised or repealed by the department.
4. Statement of Compliance pursuant to Health and Safety Code section 124977(d)(1) as to 6-6-2019 order filed 9-30-2019. Exempt from OAL review pursuant to Health and Safety Code section 124977(d)(2). Submitted to OAL for filing and printing only pursuant to Government Code section 11343.8 (Register 2019, No. 40).
