§ 631.5. Contents of Claims.
Currentness
All claims or amendments thereto shall include the name and post office address of the claimant, shall include the post office address where the claimant wants notices to be sent, shall be signed by the claimant or by a person acting on his behalf, shall be verified, and shall state the facts constituting the claim in simple concise language without resort to legal phraseology.
