§ 599.752.2. Absence Reports -- Board and Commission Members.
(a) Each pay period an employee accruing paid leave under section 599.752.1 of these regulations shall submit an Absence and Additional Time Worked Report (Std. Form 634) to the appointing authority providing personnel and payroll processing services. All absences during the pay period must be reported. If no absences occurred, a statement so indicating must be made on the form. Annual leave or vacation credits may be used for any absence. Sick leave credits may only be used as provided by Department regulations.
