Rule 27. Notice of Appeal; Current Address for Further Proceedings
(a) An appeal is commenced by filing a written notice of appeal with the trial court. The notice of appeal must identify the final order or final judgment appealed from.
(b) When the defendant is the appellant, the notice of appeal must set forth the defendant's current mailing address and phone number. Unless the court is notified in writing of a change of address, the current address must be valid for the sending of other notices to the defendant in subsequent proceedings.
(c) When a party appeals, the trial court must send a copy of the notice of appeal to the appellee.
