Rule 5. Appeals by Indigents
A party may seek a waiver of fees or costs on the basis of indigency. A request to proceed as an indigent shall be filed in the trial court on a form provided by the court containing a sworn statement of financial resources and such further information as required by the court. The court shall grant or deny the request within five business days of the request, and notify the requesting party of the court's ruling within three business days of that ruling.
