016.06.6-900. SERVICES PROVIDED
901 MANDATORY SERVICES
The Provider shall provide at least the following health care services:
901.1 Personal care such as assistance with feeding, ambulation, and toileting as needed by individual participants;
901.1 Monitoring of each participant's general health and medical regimen including screening of:
901.1.1 vital signs, once a day;
901.1.2 hygiene, once a day;
901.1.3 weight, once every 30 days;
901.1.4 dental health, once every six months;
901.2 Assistance to participants and caregivers with medical treatment plans and diets;
901.3 Injection of insulin or other IM or Sub-Q injections by licensed personnel for individual participants, as ordered by the participants' physician;
901.4 Health education programs for all participants at least once each week;
901.5 Counseling with individual participants, as needed, regarding health care;
901.6 Minor first aid treatment as needed;
901.7 Facilitating specialized services (i.e., speech therapy, physical therapy, counseling, etc.) which may be arranged for or provided through the program as needed by individual participants and as available through community sources.
902 A monthly schedule of group activities shall be planned and posted in the facility.
902.1 Activities may be conducted individually and in groups.
902.2 Activities shall be planned to suit the needs and interests of participants and designed to stimulate interest, rekindle motivation and encourage physical exercise.
902.3 Planned activities shall include but not be limited to:
902.3.1 exercise;
902.3.2 recreation;
902.3.3 social activities;
902.4 Physical exercise shall be:
902.4.1 designed in relation to each individual's needs, impairments, and abilities;
902.4.2 shall be alternated with rest periods or quiet activities.
903 Balanced, wholesome meals and supplemental snacks shall be made available in accordance with each participant's individual care plan.
903.1 Meals shall be served at approximately the same time daily.
903.2 Supplemental snacks shall consist of nourishing food and beverages.
903.3 Food may be prepared and stored on site if the requirements of the local health department and applicable regulations established by the Arkansas Department of Health are met.
903.4 Food prepared away from the site shall be prepared in a facility which meets the requirements of the local health department and applicable regulations established by the Arkansas Department of Health.
903.5 Participants will be allowed to bring their lunch from home, if they so wish.
903.6 The provider shall arrange for participants' special diets and other diet modifications as ordered by a physician.
903.7 The nutritional intake of each participant shall be observed and documented daily in the participant's record.
904 OTHER SERVICES
The Provider may offer the following services:
904.1 Transportation between the participant's home and the facility. If transportation services are offered, whether provided directly or under contract, the facility shall insure that:
904.1.1 the driver is licensed;
904.1.2 that liability insurance is in force;
904.1.3 all vehicles have seat belts;
904.1.4 all participants wear seat belts while being transported;
904.1.5 any charge for transportation shall be described in advance to the participant or responsible party.
904.2 Counseling services, whether provided directly or under contract, shall be provided by licensed or certified professional personnel.
