016.06.6-500. PARTICIPANTS
Currentness
501 ELIGIBILITY
To be eligible for ADHC, participants must;
501.1 be a functionally impaired adult;
501.2 have a written recommendation for ADHC from their physician;
501.3 have a current medical history provided;
501.4 have a written plan of care.
502 RESTRICTIONS
Participants will be deemed ineligible if they:
502.1 are bed-fast;
502.2 have behavior problems which create a hazard to themselves or others.
