016.06.54-202.210. Performing Providers of RSYC Services for Children.
Currentness
A. Rehabilitative Services for Children performing providers must complete a provider application (View or print form DMS-652) and a Medicaid contract (View or print form DMS-653) with the Arkansas Medicaid Program.
B. The performing provider of RSYC services for children must be certified by the Division of Children and Family Services as having programs and professional staff capable of delivering the rehabilitative services offered under the Arkansas State Plan.
C. The provider application and the Medicaid contract must both be approved by the Arkansas Medicaid Program.
