016.15.5-506. CLIENT MANAGEMENT

CONTROL OF CLIENTS
1. Written policy and procedure shall govern the use of control measures with clients. These measures shall be limited to the use of passive personal restraint or seclusion/time-out rooms.
2. When staff use passive physical restraint or seclusion/time-out rooms, it shall be determined and applied at the direction of a clinically privileged staff member. Either or both measures shall be discontinued upon the client attaining self control.
3. Placement of a client in a seclusion/time-out room shall not exceed 60 minutes. Staff shall conduct 15 minute checks of occupied seclusion/timeout rooms. If a client requires continued placement in excess of 60 minutes, the extended placement shall be ordered by a physician.
4. There shall be documentation for each incident involving the use of a restraint measure or a seclusion/time-out room. At a minimum the documentation shall include:
a. date, time, and description of incident which necessitated restraint or seclusion/time-out measures;
b. written order signed and dated by a clinically privileged staff member directing the use of the control measure;
c. date, time, and signature of staff member applying the restraint measure or placing the client in a seclusion/time-out room;
d. type and duration of control measure used;
e. date, time, and staff conducting required room checks;
f. any injury sustained by the client due to the control measure;
g. administrative review of the use of restraint, seclusion/time out.
