016.15.5-501. PROGRAM
Currentness
1. The facility shall develop and maintain an overall program description that contains the following components and provides for consistency in daily program operations:
a. program purpose and philosophy;
b. overview of procedures affecting clients;
c. general expectations for client behavior;
d. rules and regulations and consequences if violated;
e. program schedules;
f. description of services;
g. provisions for participation of service staff in treatment planning.
2. The facility shall provide a written program description to the client, the client's parents, or legal custodian or guardian upon admission of the client. This description shall be written in language that is age appropriate to the client.
3. The function of the residential facility shall be clearly defined. Such definitions shall include the geographical area to be served and how this area was determined, ages and types of children to be accepted for care, and the specific extent, limitation, and scope of services for which a license is being sought.
4. The residential facility shall produce evidence to licensing personnel that an annual self-evaluation of the purposes, goals, and policies has been completed, and the extent to which these purposes, goals, and policies are being accomplished.
