016.15.5-431. COMPREHENSIVE TREATMENT PLAN
1. Within 30 days of admission, a comprehensive treatment plan shall be developed and implemented. The psychiatrist must assume responsibility for planning, execution and supervision of all treatment plans.
2. The comprehensive treatment plan shall be based on all assessment data and shall be a written, time-limited, goal oriented individualized plan.
3. The comprehensive treatment plan shall be a product of the facility's interdisciplinary staffing team which shall include the following participants:
a. staff responsible for performing the assessments of the client;
b. treatment team made up of staff responsible for implementing treatment plan, direct care and treatment coordination;
c. client;
d. client's parents, or legal custodian or guardian;
e. representatives from the referring agency and/or other agencies significant to the client's case;
f. significant others as deemed necessary by the treatment plan.
Exclusion of any of the above parties from the treatment plan staffing shall be justified in writing.
4. Each client's comprehensive treatment plan shall contain the following:
a. the presenting problem(s) which shall be addressed during treatment including client strengths and any barriers to treatment;
b. a statement of goals relative to treatment, release, and aftercare, based on assessment data;
c. specific objectives to achieve the treatment goals which include a time frame and designated person responsible for each objective;
d. specific services, activities, and programs to be utilized to meet the stated goals;
e. specific criteria to be met for termination of treatment;
f. strategies planned to foster positive participation by the client's family, or legal custodian or guardian, and other agencies in the provision of treatment, discharge, and aftercare services;
g. identification of the staff member responsible for case management of the client;
h. signatures of all members of the treatment team.
5. Each treatment plan shall be reviewed by the treatment team 30 days after its initial implementation and every 60 days thereafter for the purposes of evaluating the degree to which the goals have been achieved and for making appropriate revisions, if necessary, to the plan. Treatment plans shall be reviewed more frequently whenever the plan no longer accurately reflects the client's needs.
6. The client's parents, family, or legal custodian or guardian shall be included if possible in the treatment plan review at six month intervals. If a client's length of stay is expected to be less than six months, the treatment plan review involving those parties shall be conducted at a minimum of once during the residents stay.
7. The treatment plan and all necessary revisions shall be explained to the client, his/her parents, legal custodian or guardian.
8. The treatment plan and any subsequent revisions shall be recorded in writing, signed, dated, and entered into the client's record.
